PKV · Requirements, Costs, and Comparison

Private health insurance

The cost of private health insurance depends on your age at entry, your health, and the policy. However, the quoted price says little about how much you will pay with children or in retirement. These questions belong in a private health insurance comparison.

Compare the coverage you want. The final premium and acceptance also depend on the health check.

Contract and reimbursementYour plan determines what is covered.

Outpatient treatment, hospital and teeth: The scope of services and its limits are part of the comparison.

Requirements for private health insurance

Who can switch to PKV?

A PKV tariff only replaces statutory health insurance if your insurance status allows this. As an employed person, you usually need a regular annual salary above the compulsory insurance limit. Different access rules apply to self-employed people, civil servants and students.

employees

The regular annual salary counts.

2026 your regular annual salary must generally be above 77.400 euros. A single high pay slip is not enough proof. A special limit applies to certain old stocks; You will clarify the exact status and change date with your employer and health insurance company.

Self-employed people

The premium continues even in low-income months.

Full-time self-employed people can often choose a PKV regardless of their income. Special rules apply to certain artists, journalists and farmers. First check your insurance status and then calculate the full contribution without the employer contribution.

Civil servants

The pensioner’s allowance (Beihilfe) sets the scope of remaining coverage.

With individual allowance, private health insurance often covers the remaining costs. How much that is depends on your employer and your situation. If a flat‑rate allowance is offered, also factor in the statutory health insurance.

students

This exemption is legally binding for you.

You must apply for an exemption from statutory student health insurance within three months of becoming subject to the requirement. This decision is binding for your ongoing student insurance. Compare family insurance, statutory student health insurance, and post-training contributions before making a decision.

Private health insurance: costs

What do you pay yourself?

There is no fixed standard price. Age at entry, health assessment, scope of benefits, and deductible levels determine the premium. A low-cost student plan does not guarantee that the subsequent adult plan will also be similarly priced.

In addition to the health insurance contribution, there is compulsory private nursing care insurance and, if applicable, daily sickness benefit. Also plan contributions for family members. You should only deduct a possible contribution refund once the requirements and amount have been determined.

Your employer contributes to the premium.

For employees eligible for subsidies, the employer generally pays a maximum of half of the eligible health insurance contribution, limited by the statutory maximum subsidy. Separate rules apply to compulsory long-term care insurance. You bear the deductible yourself; it is not an insurance contribution.

A 600 euro contribution does not always mean 600 euros of your own costs.

An accepted health insurance plan costs 600 € per month and has 600 € annual deductible. Select the example situation and change the eligible invoices before deducting the deductible.

Simplified example scenarios without special regulations. For employees, the full €600 is eligible for subsidy; the assumed €300 employer contribution is below the maximum subsidy for 2026. Self-employed individuals do not receive any subsidy in this example.

Bar: own contributions plus deductible, common scale up to 7.800 €. The employer does not contribute to the deductible.

Calculation: twelve own monthly contributions + the smaller amount from annual invoices and €600 deductible. In the example, any additional invoices will be fully reimbursed. Compulsory nursing care insurance, daily sick pay, dependents, uninsured services, reimbursements and taxes are not included. Your selection remains in the browser and is not saved or sent.

A high deductible requires a reserve.

You might be healthy one year and face multiple bills the next. Always budget for the full agreed deductible. Check if it applies per year, per person, or for specific services, and whether preventive care is excluded. As an employee, a lower contribution may not save you the full difference, as your employer’s contribution could also decrease.

Compare PKV tariffs

Compare private health insurance

Which benefits should your plan cover?

A single room is easy to compare. The clause about a hearing aid or longer psychotherapy is less noticeable. This is where it's worth reading: Which treatment is insured, up to what amount and with what prior agreement?

Write down your must-haves before comparing. A tariff that does not cover them should not be shortlisted simply because of its price. Neither your status as a private patient nor a high contribution guarantees you a specific doctor's appointment.

Clarify questions about medical care in conversation.

Doctor visits and therapies

Outpatient services include psychotherapy, medications, medical aids, and assistive devices. Check if a catalog of aids limits reimbursement and what commitments are required before treatment. With a primary-care tariff, visiting a specialist directly may result in reduced coverage.

GOÄ: What fee rates and fee agreements does the tariff cover?

Treatment in hospital

A single room or elective medical treatment is only insured if the tariff provides for this. Also check the reimbursement in private clinics, the free choice of hospital and the rules for rehabilitation. The room alone says little about medical protection.

View elective services and general hospital services separately.

Teeth, implants and braces

A high reimbursement rate provides little help if a low dental scale applies in the first years. Compare maximum amounts, material and lab costs, implant limits, and orthodontics. Recommended treatments should be included in the health check.

Check GOZ, sum limits and necessary treatment and cost plans.

Income during long-term illness

Health insurance pays insured treatment costs. Loss of earnings requires separate insurance through daily sick pay. For employees, the start date must coincide with the continued payment of wages; self-employed people need a solution for the time that their reserves no longer cover.

Daily sickness benefit is different from daily hospital benefit.

GOÄ and GOZ are the fee schedules for medical and dental services. The contract determines the scope of reimbursement. Legal requirements apply to adjustments to contributions and conditions; a tariff is not a blanket promise to pay every future bill in full.

GKV or PKV: What changes in everyday life?

Both systems can offer suitable protection. A PKV tariff with weak services is not made any better by the word “private”. Conversely, a good tariff can contain services that the statutory health insurance does not cover to the same extent.

Premium

GKVWithin the legal framework, it is based on income subject to contributions, up to the contribution assessment limit.

PKVDepends on tariff and insured risk at the time of conclusion. Less income does not automatically lower it.

Benefits

GKVStatutory benefit framework, supplemented by provider-specific offers. Co-payments and approvals are possible.

PKVContractually agreed scope with tariff limits. It can go beyond GKV protection, but can also contain gaps.

Family

GKVChildren and spouses or registered life partners can be insured free of charge under legal conditions.

PKVEach person with private health insurance has to pay their own contribution. The family does not necessarily have to be in the same system.

Billing

GKVMany services are billed via the health card. Personal contributions and private additional services must be paid separately.

PKVBills usually go to you. The insurer reimburses according to the tariff; Payment deadlines and outstanding amounts remain your responsibility.

Switch later

GKVChanging your health insurance fund is possible under legal regulations. Switching to private health insurance requires a suitable status.

PKVChanging tariffs and changing insurers have different consequences. Returning to statutory health insurance is not always possible.

If you only want to add individual services, a combination is also worthwhile statutory health insurance and appropriate additional insurance. For teeth, the guide explains Supplementary dental insurance the differences. Full insurance is not absolutely necessary.

PKV for children and families

Do the math for everyone who needs to be covered.

In private health insurance there is no non-contributory family insurance like in the GKV. Each privately insured child and each privately insured partner has to pay their own health insurance contribution. An offer for you alone does not answer the cost question for your family.

A combined insurance plan is possible. Whether a child can be insured for free through the legally insured parent depends on the specific conditions. For married couples or registered partners, this option is excluded if the non-legally insured parent related to the child earns more than one-twelfth of the annual salary ceiling per month and has a higher total income than the statutory health insurance member.

Have the health insurance company confirm your entitlement before you count on a child being covered without contributions.

Private health insurance for civil servants

Subsidies and tariffs must match.

With individual aid, the employer covers a fixed proportion of the expenses eligible for aid. A state aid-compliant PKV tariff insures the agreed remaining costs. The rules differ between federal and state governments; Marital status and retirement can also influence the benefit rate.

Ask your public-sector employer about a flat-rate health insurance allowance too. It is not offered everywhere and may favour voluntary statutory health insurance. Check whether this decision is binding before applying.

What also counts for civil servant candidates is the contribution that applies after the candidate's term ends. If you have pre-existing conditions, special admission measures may be relevant. You should clarify their deadlines and requirements early on.

No fixed percentages: The subsidy rate and reimbursement limits must be checked individually. Costs not eligible for subsidy are not automatically covered.

You usually receive the bill.

The cost reimbursement principle generally applies in the PKV. You will receive an invoice and submit it to the insurer. It reimburses what has to be paid according to your tariff. Direct billing is also possible for some hospital services; clarify this with the clinic and insurer beforehand.

Reimbursement in everyday lifeInvoice, refund and payment belong together.
  1. Check invoice

Do the treatment, date, and billed services match? Clarify unclear items with the practice.

  2. Submit to insurer

Via app, portal or the designated postal route. You don't always have to pay before submitting the invoice.

  3. Compare reimbursement

Check the billing for deductibles, tariff limits, and reductions. An open query does not automatically postpone the payment deadline.

  4. Pay on time

You owe the practice the invoice amount regardless of the processing status with the insurer. Plan a financial buffer for this.

Schematic sequence. Reimbursement and payment may occur at different times. There is no promise that the money will be received before the payment date.

If expensive treatment is planned, it is worth making a written promise of benefits based on the cost plan. It should fit the specific project. Telephone information does not replace checking the tariff conditions.

Health check and application

Organize your documents before switching.

The health questions are not a memory test. Request patient records or billing summaries in advance and verify them against the questions. The exact wording and timeframe matter. Omitting known issues, diagnoses, or treatments could void your policy.

In the case of pre-existing illnesses, a professionally accompanied risk inquiry can be useful before making a formal application. Clarify which data goes to whom and whether the request can be made anonymously. Depending on the findings and tariff, the insurer can request a surcharge or exclusion or reject the application.

  1. Clarify insurance status. Have your eligibility, existing contract, and possible start date checked.
  2. Compare tariffs with the same needs. Include deductible, compulsory nursing care insurance and daily sickness benefit.
  3. Await acceptance and conditions. A calculation result is not yet a binding acceptance of admission.
  4. Ensure continuous coverage Only terminate existing protection once the new coverage has been bindingly and completely clarified.

PKV in old age and when contributions increase

The premium must remain affordable on your pension.

Aging provisions are intended to help finance the higher medical costs in old age. They do not guarantee a consistent premium. With typical long‑term adult plans, the premium is not recalculated solely based on your age on each birthday. Under legal conditions, adjustments can result from changes in benefit expenditure in the insured collective.

A lower pension does not automatically make private health insurance (PKV) cheaper. Therefore, budget based on the income you are likely to have later. Also clarify any possible subsidy from the statutory pension insurance and the conditions for additional contribution relief tariffs. Their premiums should already be included in your budget today.

First check a tariff change with your own insurer.

Under Section 204 VVG, you can request a switch to other tariffs from your insurer with equivalent coverage. Acquired rights and age-related reserves are credited. For higher or more comprehensive benefits, the insurer may impose additional conditions. Compare the differences in writing, especially regarding deductibles and reimbursement limits.

Switching to another insurer is different: it may require a new medical assessment, and ageing reserves are not fully transferable at will. A lower figure in the new offer is therefore not enough reason to switch.

Frequently asked questions about private health insurance

Which private health insurance is the best?

A test winner may have been evaluated according to criteria that hardly matter to you. First, determine your minimum protection, for example for psychotherapy, aids and dentures. Then compare premiums, deductibles and conditions. The cheapest offer in the results list is not automatically the one with the lowest personal burden.

Can I take out private health insurance without undergoing a medical examination?

For regular PKV full tariffs, the health check is usually part of the admission process. Special admission conditions apply to the legally regulated basic tariff: Anyone who meets the requirements may not be rejected because of an increased health risk; Risk surcharges and corresponding service exclusions are not permitted. This is not an equivalent replacement for every service plan you want. Special admission campaigns also have their own requirements and deadlines.

Will my premium increase if I become seriously ill?

An illness that occurs after the start of the contract does not simply lead to a new personal risk surcharge in the unchanged contract. However, the insurer can adjust contributions for the affected group of insured persons under legal conditions. A distinction must be made between the consequences of incomplete information when applying or a later extension of insurance cover.

Can I go from the PKV back to the GKV?

There is no provision for free change upon request. There must be legal access, for example through new compulsory insurance. From 55 years onwards there are particularly high hurdles: § 6 Paragraph 3a SGB V takes into account, among other things, insurance in the last five years and professional status during this time. A lower salary or the end of self-employment alone does not guarantee a return. Have your health insurance company check your access before canceling.

What happens to the PKV when unemployed?

When receiving unemployment benefit, statutory insurance obligations often arise. Exceptions depend on various factors such as age, previous insurance, and possible exemption. If you remain privately insured, a limited subsidy from the provider is possible. Filing for unemployment alone does not end private insurance. Clarify insurance status, subsidies and deadlines with the employment agency and insurer.

What do the basic and standard tariffs mean?

These are legally regulated social tariffs with their own access conditions and benefits. The standard tariff is only available for certain older contracts and groups of people. The basic tariff is based on the scope of services of the GKV and has a statutory contribution limit. Neither of these are a guarantee for a low contribution in the current scope of services. Before making a change, have alternatives in the existing company examined.

Does my private health insurance also apply abroad?

This depends, among other things, on the destination, duration and tariff. Check stays outside Europe, permanent moves and return transport separately. An entitlement can secure certain rights in the event of an interruption, but does not replace ongoing health insurance coverage. Which rights are retained differs between small and large entitlement and depending on the contract.

For holiday trips you can also meet the conditions of a International travel health insurance check, especially regarding repatriation.
